Qualified Social Worker (QSW)
We are seeking a passionate and dedicated Qualified Social Worker (QSW) to join our team in Salford, specializing in Hospital and Integrated Multi-Agency Care (IMC). This role is pivotal in delivering high-quality social work services to individuals in need, ensuring their well-being and supporting their journey through complex healthcare systems. You will work closely with multidisciplinary teams to assess, plan, and implement care strategies that promote positive outcomes for clients.
Your key responsibilities will include:
- Conducting comprehensive assessments of clients' needs, strengths, and circumstances.
- Developing and implementing personalized care plans in collaboration with clients and their families.
- Working effectively with healthcare professionals, community organizations, and other stakeholders to coordinate services.
- Providing support and guidance to clients and their families regarding available resources and services.
- Maintaining accurate and up-to-date case records and documentation in compliance with regulatory standards.
To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
- Registration with Social Work England.
- Proven experience in a similar role, particularly within hospital or integrated care settings.
- Strong assessment and intervention skills, with the ability to work under pressure.
- Excellent communication and interpersonal abilities, with a commitment to client-centered practice.
