We are seeking a qualified Adult's Social Worker to join our dynamic West Derby Team. In this role, you will be responsible for providing high-quality support and intervention to vulnerable adults, ensuring their needs are met and promoting their well-being within the community.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and risks. Developing and implementing tailored care plans in collaboration with service users and their families. Providing ongoing support and advocacy to clients, ensuring access to appropriate services. Maintaining accurate and up-to-date case records, documentation, and reports. Collaborating with multidisciplinary teams, external agencies, and community organizations to enhance service delivery.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work degree or equivalent qualification. Current registration with Social Work England. Demonstrable experience in adult social work, particularly in assessment and care planning. Strong communication and interpersonal skills, with the ability to build relationships with diverse clients.
