Liverpool - Adult Social Worker - Assessment and Care Delivery

We are seeking a dedicated and compassionate Adult Social Worker to join our Assessment and Care Delivery team in Liverpool. This role is pivotal in supporting vulnerable adults to achieve their best possible quality of life. You will work collaboratively with service users, their families, and multidisciplinary teams to assess needs, develop care plans, and ensure the delivery of high-quality services that promote independence and well-being.

Your key responsibilities will include:

  • Conducting comprehensive assessments of adults in need of support, considering their physical, emotional, and social needs.
  • Developing tailored care plans in partnership with service users, ensuring their preferences and aspirations are at the forefront of service delivery.
  • Monitoring and reviewing care plans to ensure effectiveness and compliance with relevant legislation and best practices.
  • Providing guidance and support to families and carers, promoting understanding and involvement in care processes.
  • Collaborating with external agencies and community resources to enhance service delivery and support networks.

To be considered for this role, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Experience in adult social work, particularly in assessment and care delivery.
  • Strong communication and interpersonal skills, with the ability to engage effectively with diverse populations.
  • Excellent organizational skills and the ability to manage a varied caseload.
