We are seeking a qualified Adult Social Worker to join a Learning Disabilities Team supporting adults to live safe, independent and fulfilling lives. You will work with individuals, families, carers and partner agencies to assess needs, identify strengths and agree personalised support plans that promote choice, control, wellbeing and inclusion. The role will involve managing a varied caseload and responding to changing circumstances in line with relevant legislation, safeguarding procedures and professional standards.

Your responsibilities will include completing proportionate assessments, reviews and risk assessments; coordinating care and support planning; arranging and reviewing services; and promoting least restrictive, person-centred approaches. You will contribute to safeguarding enquiries, mental capacity assessments and best-interest decision-making where required. The successful candidate will work collaboratively with health professionals, housing providers, advocacy services, care agencies and other partners, while maintaining accurate, timely and confidential records. You will also support transitions, manage challenging situations constructively and ensure that individuals’ voices, rights and outcomes remain central to all decisions.

You must hold a recognised Social Work qualification and be registered with Social Work England, with relevant post-qualification experience in adult social care, ideally within learning disabilities services. A sound understanding of the Care Act, Mental Capacity Act, safeguarding responsibilities and strengths-based practice is essential. You should be confident undertaking assessments, managing competing priorities and making professional judgments, including in complex or high-risk situations. Excellent communication, report-writing and partnership-working skills are required, together with a commitment to equality, dignity and anti-discriminatory practice. A current driving licence and access to transport may be needed for community-based work. The position offers the opportunity to make a meaningful difference while developing your professional experience within a supportive multidisciplinary environment.
