Key responsibilities include:

Conducting comprehensive assessments of the needs and circumstances of older adults. Developing and reviewing care plans in collaboration with clients and their families. Providing support and guidance to clients in accessing appropriate services and resources. Working closely with multidisciplinary teams to ensure holistic care delivery. Maintaining accurate and confidential records in compliance with regulatory standards.

To be successful in this role, you must meet the following essential qualifications and skills:

Social Work degree or equivalent qualification and current registration with Social Work England. Proven experience working with older adults in a social work capacity. Strong communication and interpersonal skills, with the ability to build rapport with clients and their families. Excellent assessment and analytical skills, with a commitment to person-centred practice. Ability to work independently and collaboratively within a team environment.
