Bracknell - Adult Social Worker – CPTLD £38PH Hybrid
We are seeking a dedicated and compassionate Adult Social Worker to join our team in Bracknell, specializing in the Community and Prevention Team for Learning Disabilities (CPTLD). This hybrid position offers a competitive rate of £38 per hour, providing an excellent opportunity to make a meaningful impact in the lives of individuals with learning disabilities and their families. As a key member of our social work team, you will play a crucial role in assessing needs, planning interventions, and supporting individuals to achieve their personal goals.
Your key responsibilities will include:
- Conducting comprehensive assessments of individuals’ needs and circumstances.
- Developing and implementing care plans in collaboration with service users and their families.
- Providing ongoing support and advocacy to individuals with learning disabilities, ensuring their voices are heard.
- Working closely with multi-disciplinary teams and external agencies to facilitate effective service delivery.
- Monitoring and reviewing care plans to ensure the highest standards of care are maintained.
To be successful in this role, you must possess the following qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work or equivalent).
- Current registration with Social Work England.
- Experience working with adults with learning disabilities, preferably within a community setting.
- Strong assessment and analytical skills, with the ability to develop tailored care plans.
- Excellent communication and interpersonal skills, fostering positive relationships with service users and colleagues.
