We are seeking a qualified Social Worker to join the Hackney Preparing for Adulthood Team on a temporary basis at a competitive rate of £32 per hour. This role is pivotal in supporting young people with disabilities as they transition into adulthood, ensuring they receive the guidance and resources necessary for a successful and independent life.
Key responsibilities include:
- Conducting assessments of young people’s needs and developing tailored support plans.
- Collaborating with families, schools, and external agencies to provide comprehensive support.
- Facilitating access to training, employment, and educational opportunities.
- Monitoring progress and reviewing support plans regularly to ensure effectiveness.
- Advocating for the rights and needs of young people within the community.
The ideal candidate will possess the following qualifications and skills:
- A recognized Social Work qualification (e.g., CQSW, DipSW, or equivalent).
- Current registration with Social Work England.
- Proven experience working with young people, particularly in a transition or disability context.
- Strong communication and interpersonal skills.
