The Role
- Undertake assessments, care planning and reviews
- Coordinate support across community and hospital settings
- Work closely with individuals, families and partner agencies
- Promote recovery, independence and positive outcomes
- Manage a varied caseload within a multidisciplinary team
About You
- Qualified Social Worker (Degree/DipSW/CQSW or equivalent)
- Registered with Social Work England
- Experience working with adults, ideally within mental health services
- Strong assessment and care coordination skills
- Able to work flexibly across community and hospital settings
