Adults Social Worker | Assessment Team | Surrey County Council

Qualified Social Worker / Social Work Professional required to join a statutory Adults Service

Office base: Surrey - Redhill

Up to £37ph

Hybrid working

You must have a social work qualification and full SWE registration as well as relevant experience to be considered

Responsibilities:

Conducting assessments of individuals' needs for social care and support, considering

physical, emotional, and environmental factors.

Developing and implementing care plans tailored to everyone's circumstances, promoting

independence and well-being.

Providing advice, guidance, and practical support to individuals and their families/carers

regarding available services and resources.

Liaising with healthcare professionals, community agencies, and other stakeholders to

coordinate and deliver integrated care services.

Safeguarding vulnerable adults by identifying and responding to instances of abuse, neglect,

or exploitation, following established protocols and procedures.

Advocating for clients' rights and interests within the social care system, ensuring they

receive appropriate support and access to necessary resources.

Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in accordance with regulatory standards and organizational policies.

Participating in multidisciplinary team meetings, case conferences, and training sessions to

enhance professional development and service deliver

To be considered, it is essential that you have a recognised Social Work qualification (Social Work Degree or Diploma in Social Work) and full Social Work England registration as well as recent or significant relevant experience in a similar role.
