Job description

Immediate call for experienced Social Workers with 2 years post qualified experience for Adult Social Services within:

  • Access - Workers will be managing Emergency packages of care, safeguarding, risk assessing
  • Hospital Discharge - able to work semi-autonomously, able to cope with fast paced Hospital Discharge work - Working in a hospital setting - Safeguarding Adults - Multi disciplinary working
  • Community Adult Health Services - Assessment of Client/Carer Needs, Reviews, Mental Capacity Assessments, Support Planning, Safeguarding Enquiry, CHC Assessments, OPG/COP Work and Duty
  • Locality- older people in the community
  • Safeguarding Hubs- Experience of Duty, Assessment of Care Needs under the Care Act 2014, Carers' Assessment, Reviews, Mental Capacity Assessment, Best Interest Decision Making, Advocacy, Support Planning and Safeguarding/Risk Management is essential for this role.
  • Ideal applicants will have:

    Good understanding of strengths-based approach to maintain wellbeing and independence, promoting the use of Direct Payment and experience of NHS Checklist, DST/CHC processes and partnership working will be added advantage.

    Good experience of IT and Mosaic is essential although, training will be provided.


    Locality and Community Adult Health Services Teams require experience of joined-up service delivery enabling holistic community support, promoting residents' wellbeing and independence whilst working in partnership with families and carers.

    ASYE not essential but will be advantageous
