Transitions Social Worker (Mental Health) - Tameside

Caritas Recruitment is proud to be working with a high-performing local authority to recruit an experienced Transitions Social Worker within Adult Social Care Mental Health services. This is a fantastic opportunity to join a supportive and forward-thinking team making a real difference to young people transitioning into adulthood.
The Role
As a Transitions Social Worker, you will provide a comprehensive social care service to young people nearing adulthood who present with moderate to severe mental health needs and/or dual diagnosis including autism.
Key responsibilities include:
Managing a diverse caseload of young people transitioning from Children's to Adult ServicesUndertaking Care Act assessments and developing outcome-focused support plansCompleting funding applications and statutory/ legal work (including DoLS, COPDoL, Section 21A and court reports)Building effective relationships with individuals who may be considered hard to engage and their familiesSupporting carers in line with Care Act responsibilitiesWorking collaboratively with multi-disciplinary partners across health, education and housing Contributing to care coordination, monitoring and re-assessments to ensure evolving needs are met
You will play a key role in ensuring seamless transition pathways and positive long-term outcomes.
About You
We are looking for a confident and skilled Social Worker with:
At least 2 years post qualification experience within transitions and/or mental healthStrong knowledge of SEND and EHCP processesExperience working with young people transitioning to adult servicesSound knowledge of relevant legislation and safeguarding practice Essential RequirementsSocial Work Degree or DipSWCompleted ASYERegistration with Social Work EnglandFull UK driving licence and access to a vehicleSkills & Experience

Strong analytical, assessment and report-writing skillsAbility to manage complex cases and make evidence-based decisionsExcellent communication and negotiation abilitiesConfidence working in challenging situations and managing conflictStrong organisational skills with the ability to prioritise workloadA person-centred, anti-oppressive approach to practice Salary & BenefitsSalary range: £39,862 - £45,091 (Grade H+)Up to 31 days annual leave + bank holidaysFlexible working, including 1 day WFH per weekAccess to Local Government Pension SchemeStructured supervision, peer support and ongoing trainingClear development pathways including AMHP training opportunitiesAdditional AMHP payments (£82 per shift, approx. 25-30 shifts per year)Working PatternFull-time: 36 hours per weekMonday - Friday (with occasional Saturdays)Flexible working to support service delivery

Caritas Recruitment is supporting Tameside Council to recruit 3 Permanent Mental Health Social Workers to join their Adult Community Mental Health Teams.
Salary
£39,862 - £45,091 (SCP 29-34, Grade H+)
Additional £82 per shift for AMHP rota (approx. 25-30 shifts per year)AMHP training available if not already qualifiedLocation
Tameside (Community Mental Health Teams - based at Wilshaw House)
About the Role
This is an exciting opportunity to work within a multi-disciplinary Adult Mental Health team, supporting adults of working age experiencing moderate to severe mental health needs.
You will:
Complete Care Act assessments and develop clear, outcome-focused support plansUndertake complex work including DoLS, COPDOL, Section 21A, and court reportsWork collaboratively with health professionals and partner agenciesAssess and manage risk while promoting independence and wellbeingBuild effective relationships with individuals who may be harder to engageSupport carers and contribute to safeguarding processesWork with young people transitioning into Adult Mental Health servicesParticipate in the duty rota and respond to crisis situations when required
There is also the opportunity to participate in or train for the AMHP rota, supporting statutory mental health interventions.

The Role
You will take a lead role in managing and delivering Section 117 aftercare responsibilities, ensuring individuals receive effective, person-centred support following discharge from hospital.
Working within a multi-disciplinary environment, you will promote independence, ensure statutory compliance, and support positive long-term outcomes through strengths-based practice.
