We provide support for people at times of crisis, additional intermediate need or following a stay in hospital. We also deliver technology enabled care and provide equipment and adaptations for people in the place they call home. It's a short-term service focused on promoting recovery, preventing unnecessary hospital admissions, and reducing reliance on long-term care facilities. This type of care supports people to regain their independence and return to their previous level of functioning.
