We are currently seeking a dedicated Adult's Social Worker to join our Locality Team in North Lincolnshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults in the local community.

Key responsibilities and duties include:

Conducting assessments and developing care plans for adults in need of support Working collaboratively with multi-disciplinary teams to ensure the best outcomes for service users Providing advocacy and support to vulnerable adults to safeguard their well-being Participating in case conferences and reviews to monitor progress and make necessary adjustments to care plans Keeping accurate and up-to-date records of interventions and outcomes

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC) Demonstrated experience in working with adults in a social care setting Strong communication and interpersonal skills to engage effectively with service users and stakeholders Ability to work autonomously and make sound decisions in complex situations Knowledge of relevant legislation and policies related to adult social care
