Caritas Recruitment is currently recruiting for an experienced Social Worker to join the Preparing for Adulthood Team (PfA) at the London Borough of Bromley.
This rewarding role sits within Bromley's 0-25 Service and focuses on supporting young people aged 17½ to 25 with learning disabilities, autism and additional needs as they transition into adulthood.
Key Responsibilities
Undertake Care Act assessments, reviews and support planning.Complete Mental Capacity Act assessments and Best Interests decisions.Manage a caseload of young people transitioning into adult services.Carry out safeguarding enquiries and risk management activity.Work closely with families, health professionals, education providers and partner agencies.Participate in Court of Protection and Best Interests work where required.Maintain accurate assessments, reports and case records.About You
Qualified Social Worker registered with Social Work England.Strong Adult Social Care experience.Sound knowledge of the Care Act 2014 and Mental Capacity Act 2005.Experience undertaking assessments, support planning and safeguarding work.Ability to work effectively within a multi-agency environment.Desirable Experience
Preparing for Adulthood or transition work.Supporting young adults with learning disabilities and/or autism.Court of Protection experience.Why Apply?
