We are seeking a compassionate, skilled, and motivated individual to support vulnerable adults, ensuring they receive the care and assistance they need to live fulfilling lives. You will work collaboratively with service users, families, and multidisciplinary teams to assess needs, develop care plans, and advocate for effective service delivery.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals’ needs and risks. Developing, implementing, and reviewing person-centered care plans. Providing direct support and guidance to individuals and their families. Collaborating with healthcare providers, community organizations, and other stakeholders. Maintaining accurate records and documentation in accordance with legal and organizational standards.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Experience in adult social work, particularly within community settings. Strong communication and interpersonal skills, with the ability to build rapport with service users.
