Key ResponsibilitiesManage a caseload focused on hospital discharges and reassessmentsUndertake Care Act assessments, reviews, and capacity assessmentsLiaise with multidisciplinary teams, including NHS colleagues and community servicesSupport individuals and families to achieve safe and sustainable discharge outcomesApply a strengths-based, person-centred approach in line with best practiceWhat We're Looking ForQualified Social Worker (Social Work England registered)Experience within hospital discharge teams or similar adult services settingsStrong knowledge of the Care Act, MCA, and safeguarding frameworksAbility to work flexibly across various hospital sites in East YorkshireExcellent communication and partnership-working skillsWhy Join This Team?Competitive rate of £33.24 per hourLong-term contracts offering stability and continuityOpportunity to be part of an authority undergoing positive transformationSupportive leadership and collaborative team environmentValuable experience across a range of hospital settings
