Join our dedicated Young Adults (Transitions) Team in Herefordshire as a Qualified Social Worker, where you will play a vital role in supporting young adults as they transition into independent living. In this rewarding position, you will work collaboratively with individuals, families, and other professionals to ensure the best outcomes for young adults with diverse needs. Your expertise will help empower them to achieve their goals and enhance their quality of life.

Your key responsibilities will include:

Conducting comprehensive assessments of young adults' needs and strengths. Developing and implementing tailored support plans in collaboration with service users and their families. Providing ongoing support and guidance to young adults during their transition to independence. Working closely with partner agencies to coordinate services and resources. Maintaining accurate and up-to-date records in line with social work standards and legislation.

To be successful in this role, you will need to possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, CQSW). Registration with Social Work England. Experience working with young adults, particularly in transition settings. Strong communication and interpersonal skills, with the ability to build rapport with service users and their families. Excellent organizational skills and the ability to manage a varied caseload effectively.

Are you a passionate and dedicated social worker looking to make a real difference in the lives of young adults? Herefordshire is seeking a qualified Social Worker to join our Young Adults (Transitions) Team. In this rewarding role, you will work collaboratively with young people and their families, ensuring a smooth transition into adulthood and supporting their individual needs.

Your key responsibilities will include:

Conducting assessments to identify the needs and aspirations of young adults transitioning from children’s services. Developing and implementing tailored support plans in partnership with young adults, families, and other professionals. Providing guidance and support to young adults in areas such as education, employment, housing, and health. Monitoring and reviewing the effectiveness of support plans, making adjustments as necessary. Maintaining accurate records and documentation in line with statutory requirements and best practice standards.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (BA/MA in Social Work or equivalent). Registration with Social Work England. Experience working with young adults, particularly in a transition or support role. Strong communication and interpersonal skills, with the ability to build rapport and trust. Knowledge of relevant legislation, policies, and best practices in social work.
