Your key responsibilities will include:
Conducting comprehensive assessments of young adults' needs and strengths. Developing and implementing tailored support plans in collaboration with service users and their families. Providing ongoing support and guidance to young adults during their transition to independence. Working closely with partner agencies to coordinate services and resources. Maintaining accurate and up-to-date records in line with social work standards and legislation.To be successful in this role, you will need to possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, CQSW). Registration with Social Work England. Experience working with young adults, particularly in transition settings. Strong communication and interpersonal skills, with the ability to build rapport with service users and their families. Maintaining accurate records and documentation in line with statutory requirements and best practice standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (BA/MA in Social Work or equivalent). Registration with Social Work England. Experience working with young adults, particularly in a transition or support role.
