Join our dedicated Leaving Care Team in Hackney as a Qualified Social Worker specializing in Children's Services. We are seeking a passionate and committed individual who will make a positive impact on the lives of young people transitioning from care to independent living. In this role, you will work closely with young people aged 16-25, providing support, guidance, and advocacy to ensure their successful transition into adulthood.
Your key responsibilities will include:
Conducting comprehensive assessments of young people's needs and developing tailored support plans. Building trusting relationships with young people, empowering them to express their views and make informed decisions. Coordinating and facilitating access to housing, education, employment, and health services. Maintaining accurate and up-to-date case records and documentation in compliance with statutory requirements. Collaborating with multi-agency partners to ensure holistic support for young people.To be considered for this role, you must possess:
A recognized social work qualification (BA/MA in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with young people, particularly in a leaving care or related context. Strong assessment and analytical skills, with the ability to manage complex cases effectively.
