We are seeking a dedicated and compassionate Adult's Social Worker to join our Hospital Team in Lewisham. In this crucial role, you will work collaboratively within a multidisciplinary team to provide high-quality social care services to adults in a hospital setting. Your primary focus will be on assessing, planning, and implementing care plans that promote the well-being and independence of our clients.

Key responsibilities include:

Conducting comprehensive assessments of clients' needs, risks, and strengths. Developing and implementing person-centered care plans in collaboration with clients and their families. Providing emotional support and practical advice to clients and their families throughout the hospital stay. Working closely with medical staff, other social workers, and community agencies to ensure seamless discharge planning. Maintaining accurate and timely documentation of assessments, interventions, and outcomes.

To be considered for this position, you must possess the following qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Proven experience in adult social work, preferably within a hospital or health care setting. Strong communication and interpersonal skills, with the ability to engage effectively with clients and professionals. Excellent organizational skills and the ability to manage a caseload effectively.
