The City of Wolverhampton is seeking a dedicated and compassionate Qualified Social Worker to join our Ageing Well Team. This role is vital in supporting older adults in our community to live fulfilling and independent lives. As a key member of our team, you will work collaboratively with individuals, families, and various stakeholders to provide high-quality social care services that promote wellbeing and safeguard vulnerable adults.
Key responsibilities of the role include:
Conducting assessments of individuals' needs and developing tailored support plans. Providing direct support and guidance to older adults and their families, ensuring their voices are heard. Collaborating with healthcare professionals, community organizations, and other agencies to coordinate care and resources. Monitoring and reviewing care plans to ensure they remain effective and relevant. Promoting independence and access to community resources, including social activities and support services. Maintaining accurate and up-to-date case records in compliance with local policies and procedures.To be considered for this position, candidates must possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience in adult social care, particularly with older adults. Strong assessment and analytical skills, with the ability to develop effective care plans. Excellent communication and interpersonal skills, demonstrating empathy and understanding.
