The Role
As part of the Aging Well Adults' Community Team, you will support adults across the borough to maintain independence, wellbeing, and strong community connections through a strengths-based approach.
You will undertake assessments, develop personalised care plans, and work collaboratively with health professionals, community partners, and families to ensure individuals receive the right support at the right time.
