In this vital role, you will work collaboratively with individuals, families, and other professionals to assess, plan, and implement tailored interventions that promote independence and well-being. Your expertise will be essential in safeguarding vulnerable adults and ensuring their rights and needs are prioritized within the community.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals to determine eligibility for services and support. Developing and implementing person-centered care plans in collaboration with service users and their families. Providing expert advice and guidance on mental health legislation, safeguarding policies, and best practices in social work. Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in accordance with local and national standards. Participating in multi-disciplinary team meetings and contributing to the overall strategic direction of services.To be successful in this role, you will need to possess the following essential qualifications and skills:
A relevant social work qualification (Degree in Social Work or equivalent). Registered with Social Work England as an Approved Social Worker. Strong knowledge of mental health legislation and safeguarding practices. Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
