Join our dedicated Community Team as a Social Worker in beautiful West Sussex, where you will play a pivotal role in enhancing the lives of individuals and families within the community. Your primary responsibility will be to assess, plan, and implement support for those in need, ensuring their welfare and safeguarding. You will work collaboratively with various stakeholders, including families, healthcare providers, and other social services, to create effective care plans tailored to individual circumstances.
Key responsibilities include:
Conducting assessments to identify client needs and risks. Developing and implementing care plans in partnership with clients and their families. Providing ongoing support and monitoring the effectiveness of interventions. Maintaining accurate records and documentation in compliance with legal and organizational standards. Participating in multi-disciplinary meetings and contributing to case discussions.To be considered for this position, you must hold a recognized social work qualification (e.g., BA or MA in Social Work) and be registered with Social Work England. Additionally, you should possess strong interpersonal skills, a compassionate approach to client care, and the ability to work effectively within a team. Experience in community social work, particularly with vulnerable populations, is highly desirable.
