Are you a dedicated and compassionate social worker with a passion for supporting individuals with learning disabilities? Join our dynamic team in Somerset as a CHC Social Worker specializing in Learning Disabilities. In this rewarding role, you will play a pivotal part in enhancing the lives of individuals by providing high-quality assessments, support planning, and advocacy.

Your key responsibilities will include:

Conducting comprehensive assessments of individuals with learning disabilities to determine their needs and eligibility for services. Developing and implementing personalized care plans in collaboration with clients, families, and multidisciplinary teams. Providing ongoing support and guidance to individuals and their families, ensuring they have access to appropriate resources and services. Advocating for clients' rights and needs within various settings, including health and social care systems. Maintaining accurate and up-to-date case records in compliance with local and national regulations.

To succeed in this role, you will need the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Proven experience working with individuals with learning disabilities or in a similar field. Strong communication and interpersonal skills, with the ability to build rapport with clients and their families. Knowledge of relevant legislation and best practices in social work and learning disabilities.
