Are you a passionate and dedicated Social Worker looking to make a meaningful impact in the lives of young people transitioning to adulthood? Rotherham Council is seeking a qualified Social Worker to join our Preparing for Adulthood Team. In this role, you will work collaboratively with young individuals aged 14-25, supporting them through the challenges of transitioning from childhood to adulthood and helping them achieve their goals and aspirations.

As a Social Worker within our team, your key responsibilities will include:

Conducting assessments and developing tailored support plans for young people. Providing guidance and advocacy to empower young individuals and their families. Collaborating with multi-agency partners to ensure a holistic approach to care and support. Monitoring and reviewing care plans, ensuring services are responsive to changing needs. Facilitating workshops and support groups to enhance life skills and independence.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (BA/MA in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with young people, particularly in transition planning. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Effective organizational skills and the ability to manage a caseload efficiently.
