We are seeking a dedicated and compassionate Social Worker to join our Preparing for Adulthood Team in Bromley. This role is ideal for individuals passionate about empowering young people with special educational needs and disabilities as they transition into adulthood. The successful candidate will play a crucial role in providing support, guidance, and advocacy to help these individuals achieve their goals and lead fulfilling lives.

Key responsibilities include:

Conducting assessments to understand the needs and aspirations of young people and their families. Developing and implementing personalized support plans that facilitate a smooth transition to adulthood. Collaborating with educational institutions, healthcare providers, and community organizations to ensure comprehensive support. Advocating for the rights and welfare of young people, ensuring they have access to necessary resources and opportunities. Maintaining accurate documentation and case records in compliance with local and national policies.

To be considered for this role, candidates must possess:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience working with young people, particularly those with special educational needs and disabilities. Strong communication and interpersonal skills, with the ability to build rapport and trust with young people and their families. Excellent organizational skills and the ability to manage a diverse caseload effectively.
