Are you a passionate and dedicated social worker looking to make a significant impact in the lives of young people transitioning out of care? The Leaving Care Team in Hackney is seeking a qualified Social Worker to join our dynamic team. In this role, you will work closely with young adults aged 16-25, providing support and guidance as they navigate the challenges of independent living.
Your key responsibilities will include:
Conducting assessments and developing personalized care plans for young people. Providing direct support and advocacy to ensure the best outcomes for each individual. Collaborating with external agencies, including housing services and educational institutions. Facilitating life skills training and workshops to empower young adults. Maintaining accurate and up-to-date case records in line with local and national standards.To be successful in this role, you must possess the following essential qualifications and skills:
A relevant social work qualification (e.g., Degree in Social Work, DipSW, CQSW). Current registration with Social Work England. Experience in working with young people, particularly in a leaving care context. Strong communication and interpersonal skills, with the ability to build rapport.
