We are seeking a dedicated and compassionate Social Worker to join our Hospital Discharge team in Poole, Bournemouth. This role is essential in ensuring a smooth transition for patients returning home or moving to alternative care settings after their hospital stay. You will work closely with patients, families, and healthcare professionals to assess needs, develop discharge plans, and provide ongoing support. Key responsibilities include:

Conducting assessments of patients' social, emotional, and practical needs prior to discharge. Developing and implementing discharge plans that align with patients' best interests. Collaborating with multidisciplinary teams to ensure continuity of care and effective communication. Providing information and support to patients and their families regarding available services and resources. Advocating for patients' rights and ensuring their voices are heard in the discharge process.

To be successful in this role, candidates must possess the following qualifications and skills:

A recognized degree in Social Work and current registration with Social Work England. Proven experience in hospital discharge planning or a related field. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Excellent problem-solving and organizational skills, with a keen attention to detail. Empathy and resilience in dealing with challenging situations.
