Join our dedicated team at Derbyshire as a Social Worker in the Adult Care Assessment and Triage Team (ACATT). We are seeking a passionate and skilled individual to provide high-quality social work services to adults in need of care and support. In this role, you will be responsible for assessing the needs of clients, developing care plans, and facilitating access to appropriate services. Your expertise will play a crucial role in enhancing the well-being and quality of life for vulnerable adults in our community.
Your key responsibilities will include:
Conducting comprehensive assessments of clients’ social, emotional, and practical needs. Developing and implementing individualized care plans in collaboration with clients and their families. Providing information and advice to clients regarding available services and support options. Collaborating with multidisciplinary teams to ensure a holistic approach to care. Maintaining accurate and up-to-date records and documentation in accordance with legal and organizational standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work or equivalent). Registration with Social Work England. Experience in adult social care, particularly in assessment and triage. Strong communication and interpersonal skills, with the ability to build rapport with clients.
