We are seeking a dedicated and compassionate Social Worker for the A&C - OC Cognitive and Physical Disability team in Leicestershire. This role is essential in supporting individuals with cognitive and physical disabilities, ensuring they receive the care and support they need to thrive. You will be responsible for conducting assessments, developing care plans, and collaborating with multidisciplinary teams to provide holistic support to clients and their families.
Your key responsibilities will include:
Conducting comprehensive assessments of clients' needs and circumstances. Developing and implementing personalized care plans in collaboration with clients and their families. Providing ongoing support and advocacy to individuals with cognitive and physical disabilities. Collaborating with healthcare professionals, community organizations, and local authorities to ensure seamless service delivery. Maintaining accurate and up-to-date case records and documentation.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW). Registration with Social Work England. Proven experience working with individuals with cognitive and physical disabilities. Strong communication and interpersonal skills, with the ability to build trusting relationships.
