Join our dedicated Community Learning Disability Team in Salford as an Adults Advanced Practitioner. We are seeking a passionate and experienced professional to lead and support our efforts in providing high-quality care and services to adults with learning disabilities. This role offers an exciting opportunity to make a significant impact in the lives of individuals and their families, ensuring they receive the support they need to thrive in their communities.

Key responsibilities include conducting comprehensive assessments, developing and implementing person-centered care plans, and collaborating with multidisciplinary teams to ensure holistic support. You will also provide expert guidance and supervision to junior staff and contribute to the continuous improvement of service delivery. Your role will involve engaging with service users, families, and external stakeholders to advocate for their needs and rights, ensuring that our services are accessible and effective.

To be successful in this role, you must possess the following essential qualifications and skills:

A relevant professional qualification in health or social care (e.g., Social Work, Occupational Therapy, or Nursing) Significant experience working with adults with learning disabilities Strong assessment and care planning skills Excellent communication and interpersonal abilities Leadership experience and the ability to mentor and support junior team members
