We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team at a leading hospital in Portsmouth. As a QSW in this hospital setting, you will play a crucial role in providing support and assistance to patients and their families during their stay, ensuring their well-being and facilitating their transition back into the community.

Key responsibilities and duties:

Conduct assessments to determine the needs and risks of patients, creating tailored care plans. Provide emotional support and counseling to patients and their families, addressing any concerns or issues they may have. Collaborate with medical staff, psychologists, and other healthcare professionals to ensure a holistic approach to patient care. Advocate for patients' rights and access to necessary services, resources, and support systems. Participate in multidisciplinary team meetings and case conferences to review and evaluate patient progress.

Essential qualifications and skills:

Qualified Social Work (QSW) certification and registration with the Health and Care Professions Council (HCPC). Demonstrated experience in a hospital or healthcare setting, with knowledge of relevant policies and procedures. Strong communication and interpersonal skills, with the ability to build rapport and trust with diverse individuals. Excellent assessment and decision-making abilities, with a focus on promoting positive outcomes for patients. Ability to work effectively under pressure, prioritize tasks, and manage a caseload efficiently.
