Join our dedicated team in North Lincolnshire as a Social Worker in the Adults Community sector. As a Social Worker, you will be responsible for assessing the needs of clients, developing and implementing care plans, and providing ongoing support to individuals and their families.
Your key responsibilities will include:
Conducting comprehensive assessments of clients' needs and circumstances. Developing tailored care plans in collaboration with clients, families, and other professionals. Providing guidance and support to clients to access community resources and services. Monitoring and reviewing care plans to ensure effectiveness and adapting as necessary. Working closely with multidisciplinary teams to ensure a holistic approach to care.To be considered for this position, you must possess the following essential qualifications and skills:
A recognized social work qualification (BA/MA in Social Work or equivalent). Registration with Social Work England. Proven experience in adult social work, preferably within community settings. Strong assessment and analytical skills.
