We are seeking a compassionate and dedicated Social Worker to join our Community Mental Health Team in Newcastle. This role is pivotal in providing support to individuals facing mental health challenges, ensuring they receive the necessary resources and interventions to improve their quality of life. As a Social Worker in our team, you will work collaboratively with multidisciplinary professionals to deliver effective care and support to our clients.
Your key responsibilities will include:
Conducting comprehensive assessments of clients’ needs and developing tailored care plans. Providing direct support and counseling to individuals and families, promoting mental well-being and recovery. Coordinating with healthcare providers, community organizations, and support services to facilitate holistic care. Monitoring clients’ progress and adjusting care plans as necessary to ensure optimal outcomes. Advocating for clients' rights and access to resources, ensuring their voices are heard in the care process.To be considered for this role, you must possess the following qualifications and skills:
A recognized qualification in Social Work (e.g., BA/MA in Social Work). Current registration with the relevant professional body (e.g., Social Work England). Experience working in mental health settings, ideally within community services. Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
