Join our dedicated team in Liverpool as a Social Worker specializing in Assessment and Care Delivery. We are seeking a compassionate and skilled individual to support vulnerable individuals and families, ensuring their needs are met through comprehensive assessments and tailored care plans. This role is pivotal in enhancing the well-being and quality of life for those we serve within the community.
Key responsibilities include:
Conducting thorough assessments of individuals and families to identify needs and risks. Developing and implementing personalized care plans in collaboration with clients and multidisciplinary teams. Providing ongoing support and advocacy, ensuring that clients have access to necessary services and resources. Monitoring and reviewing care plans regularly to adapt to changing circumstances and needs. Maintaining accurate and up-to-date records in compliance with local and national regulations.To excel in this role, you will need to possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, CQSW, or equivalent). Registration with Social Work England. Experience in assessment and care delivery within a social work context. Strong communication and interpersonal skills, with the ability to build rapport with clients and collaborate effectively with colleagues.
