We are seeking a dedicated and compassionate Adult Social Worker to join our Assessment and Care Delivery Team in Liverpool. In this vital role, you will be responsible for supporting vulnerable adults in navigating complex social care needs. You will work collaboratively with individuals, families, and multidisciplinary teams to assess needs, develop care plans, and ensure the delivery of high-quality services that promote independence and well-being.
Your key responsibilities will include:
Conducting comprehensive assessments of adults’ needs and strengths. Developing, implementing, and reviewing care plans in collaboration with service users and their families. Providing ongoing support, guidance, and advocacy to clients and their families. Working closely with other professionals and agencies to ensure a coordinated approach to care. Maintaining accurate and up-to-date records in accordance with legal and organizational standards.The ideal candidate will possess the following essential qualifications and skills:
A degree in Social Work or equivalent qualification recognized by Social Work England. Registration with Social Work England. Experience working within adult social care, particularly in assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds.
