Are you a passionate and dedicated social worker looking to make a meaningful impact in the lives of individuals with learning disabilities? We are seeking a qualified Learning Disability / Transitions Social Worker to join our dynamic team in Coventry. In this role, you will play a crucial part in supporting individuals during pivotal life transitions, ensuring they receive the necessary resources and guidance to thrive in their communities.

Your key responsibilities will include:

Conducting comprehensive assessments to determine the needs of individuals with learning disabilities. Developing and implementing personalized support plans in collaboration with service users and their families. Facilitating transitions from educational settings to adulthood, including employment and independent living. Working closely with multidisciplinary teams, community organizations, and statutory agencies to provide holistic support. Monitoring progress and reviewing support plans regularly to ensure effectiveness and adapt to changing needs.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with individuals with learning disabilities and knowledge of relevant legislation. Excellent communication and interpersonal skills, with the ability to build trusting relationships. Strong organizational skills and the ability to manage a varied caseload effectively.
