We are seeking a passionate and dedicated Qualified Social Worker (QSW) to join our Children with Disabilities (CWD) Team in Watford, Hertfordshire. In this rewarding role, you will work closely with children and young people aged 0-25 who have disabilities, ensuring they receive the necessary support and services to thrive. You will be part of a dynamic team committed to promoting the welfare and development of vulnerable children and their families.
Your key responsibilities will include conducting assessments, developing care plans, and providing ongoing support to children and families. You will collaborate with various stakeholders, including health professionals, educators, and community organizations, to ensure a holistic approach to care. Additionally, you will be responsible for maintaining accurate case records and adhering to safeguarding policies.
Conduct comprehensive assessments of children and families to identify needs and develop tailored support plans. Provide guidance and support to families, ensuring they are informed about available resources and services. Work collaboratively with multi-disciplinary teams to coordinate services and interventions. Monitor and review the progress of children and families, making adjustments to care plans as necessary. Participate in training and professional development opportunities to enhance your skills and knowledge.To be considered for this role, you must hold a relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent) and be registered with Social Work England. Experience working with children with disabilities is essential, along with a strong understanding of relevant legislation and safeguarding policies. Excellent communication, organizational, and interpersonal skills are critical to ensure effective collaboration with families and professionals.
