The South and West Herts Older People & Sensory Services Team is seeking a dedicated and compassionate Qualified Social Worker to join our dynamic team. In this role, you will play a pivotal part in supporting older adults and individuals with sensory impairments, ensuring they receive the highest standard of care and support tailored to their unique needs. Your expertise will be vital in promoting independence, enhancing quality of life, and safeguarding vulnerable populations within our community.
Key responsibilities include:
Conducting comprehensive assessments of clients' needs and developing tailored care plans. Collaborating with healthcare professionals, families, and community organizations to coordinate services. Providing ongoing support and guidance to clients and their families, ensuring they understand available resources. Monitoring and reviewing care plans regularly to adapt to clients' changing needs. Advocating for clients' rights and well-being, ensuring their voices are heard in decision-making processes.To be considered for this position, candidates must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience working with older adults and/or individuals with sensory impairments. Strong communication and interpersonal skills, with the ability to build rapport with clients and families. Excellent organizational skills and the ability to manage a caseload effectively.Additional requirements include a commitment to continuous professional development and a valid driving license to facilitate community visits.
