As an Adult's Social Worker within our Locality Team, you will be responsible for a variety of key duties, including:
Conducting assessments of individuals’ needs and risks to develop tailored support plans. Working collaboratively with multi-disciplinary teams and external agencies to ensure comprehensive care delivery. Providing direct support and interventions to adults facing complex challenges, including mental health issues, disabilities, and social isolation. Maintaining accurate and up-to-date case records in compliance with regulatory standards. Advocating for clients’ rights and ensuring they have access to necessary resources and services.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Proven experience in adult social work, particularly within locality teams. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
