Key responsibilities include:
Conducting comprehensive assessments of patients' social, emotional, and practical needs. Developing and implementing discharge plans in collaboration with healthcare professionals, patients, and their families. Providing information and support to patients and families to help them navigate available resources and services. Advocating for patients’ rights and ensuring their voices are heard in decision-making processes.
