We are seeking a dedicated and experienced Qualified Social Worker to join our Rotherham Adults Access Point (AP) Front Door Service on a 6-month contract. This hybrid position offers a competitive rate of £36.50 per hour, allowing you to make a significant impact within the community while enjoying the flexibility of remote work. As part of our dynamic team, you will play a vital role in assessing and responding to the needs of vulnerable adults, ensuring their safety and well-being.

Your key responsibilities will include:

Conducting thorough assessments of adult clients to identify their needs and risks. Collaborating with various stakeholders, including health services, community organizations, and families. Developing and implementing care plans that promote independence and enhance quality of life. Providing advice and support to clients and their families, ensuring they understand available services. Maintaining accurate and up-to-date records in accordance with local policies and procedures.

To be considered for this role, you must possess:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Demonstrable experience in adult social work, particularly within front door services. Excellent communication and interpersonal skills, with the ability to build rapport with clients. Strong analytical and problem-solving abilities, with a commitment to safeguarding vulnerable adults.
