Advocating for clients' rights and ensuring compliance with statutory requirements and best practices. Providing guidance and support to families, helping them navigate the transition process.

The ideal candidate will possess essential qualifications and skills, including:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with adults, particularly in transition settings. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Knowledge of relevant legislation and policies affecting adult social care.
