Join our dedicated Long Term Team in Rhondda Cynon Taf as an Adult's Social Worker! We are seeking a passionate and qualified individual to make a meaningful impact in the lives of adults within our community. This role will provide you with the opportunity to work collaboratively with individuals, families, and other professionals to deliver high-quality social care services that promote independence and well-being.

Your key responsibilities will include:

Conducting assessments to identify the needs of adults and developing tailored support plans. Providing ongoing support and guidance to clients, ensuring their voices are heard and respected. Collaborating with multi-disciplinary teams to ensure comprehensive care and support. Maintaining accurate and up-to-date case records in compliance with statutory requirements. Participating in training and development opportunities to enhance your professional skills.

The ideal candidate will possess the following qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent). Registration with Social Care Wales. Demonstrated experience in adult social work, particularly in a long-term care setting. Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues. Proficiency in assessment and care planning processes.

Join the dedicated Adult's Social Work - Learning Disabilities Team in Rhondda Cynon Taf as a Qualified Social Worker. We are seeking a passionate and skilled professional committed to improving the lives of individuals with learning disabilities. In this rewarding role, you will work collaboratively with service users, their families, and multi-agency partners to deliver high-quality social care and support tailored to individual needs.

Your key responsibilities will include:

Conducting assessments of need and risk to develop comprehensive care plans. Providing ongoing support and advocacy for individuals with learning disabilities. Collaborating with healthcare providers, community organizations, and families to ensure holistic care. Monitoring and reviewing care plans to ensure effectiveness and compliance with relevant policies. Participating in team meetings and contributing to service development initiatives.

To be considered for this role, you must possess the following essential qualifications and skills:
