Join our dedicated Long Term Team in Rhondda Cynon Taf as an Adult Social Worker, where you will play a vital role in enhancing the lives of vulnerable adults within our community. In this rewarding position, you will be responsible for assessing the needs of service users, developing and implementing care plans, and ensuring that individuals receive the support they require to lead fulfilling lives. Your expertise will help to promote independence and safeguard the wellbeing of adults, while also working collaboratively with families and other professionals.
Your key responsibilities will include:
Conducting comprehensive assessments to identify individual needs and risks. Developing and reviewing care plans in partnership with service users and their families. Providing support and guidance to individuals, helping them access necessary services. Maintaining accurate records and documentation in compliance with relevant legislation. Participating in multi-disciplinary team meetings and contributing to service development.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized Social Work qualification (BA/MA Social Work or equivalent). Registration with Social Care Wales. Experience in adult social work, particularly in long-term support settings. Strong communication and interpersonal skills to build rapport with service users.
