Join our dedicated Adult's Social Work team in Surrey, specializing in Older People and Physical Disabilities (OP/PD). We are seeking a passionate and qualified Social Worker to make a real difference in the lives of vulnerable adults. In this role, you will work collaboratively with individuals, families, and various agencies to assess needs, develop care plans, and provide ongoing support to promote independence and well-being.
Your key responsibilities will include:
Conducting comprehensive assessments of adults with physical disabilities and older individuals to determine their needs and eligibility for services. Developing, implementing, and reviewing personalized care plans in collaboration with service users and their families. Providing information, advice, and support to individuals and their families, ensuring they are empowered to make informed decisions. Working closely with healthcare professionals, community organizations, and other stakeholders to coordinate services and resources. Maintaining accurate records and documentation in compliance with organizational policies and regulatory requirements.To be successful in this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work or equivalent). Current registration with Social Work England. Proven experience in adult social work, particularly with older adults and those with physical disabilities. Strong assessment and analytical skills, with the ability to develop effective support plans.
