Join our dedicated Integrated Care & Public Health Team as an Adult's Social Worker, where you will play a vital role in enhancing the well-being of individuals within our community. In this position, you will work collaboratively with multidisciplinary teams to deliver high-quality support and care to adults facing a range of challenges. Your expertise will be essential in assessing needs, developing care plans, and ensuring that individuals receive the appropriate services and resources to thrive.
Your key responsibilities will include:
Conducting comprehensive assessments to identify individual needs and risks. Developing, implementing, and reviewing care plans in partnership with clients and their families. Providing ongoing support and advocacy to clients, ensuring their voices are heard in decision-making. Collaborating with healthcare providers, community organizations, and other stakeholders to coordinate services. Monitoring and evaluating the effectiveness of interventions and adjusting plans as necessary.To be successful in this role, you will need the following essential qualifications and skills:
A recognized degree in Social Work and registration with the relevant professional body. Demonstrable experience in adult social work, particularly within integrated care and public health settings. Strong assessment and analytical skills with the ability to develop tailored care plans. Excellent communication and interpersonal skills, with a commitment to client-centered practice.
