Your key responsibilities will include:
Conducting comprehensive assessments to determine the needs of adults requiring social care services. Developing and implementing personalized care plans in collaboration with clients and their families. Providing ongoing support and advocacy to clients, ensuring their voices are heard and respected. Working closely with multidisciplinary teams, including healthcare professionals, to coordinate services and resources. Maintaining accurate records and documentation in compliance with regulatory standards.To be considered for this position, you must possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work or equivalent). Current registration with the relevant social work regulatory body. Proven experience working with adults in a social care setting. Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
