Are you an experienced practitioner looking to make a significant impact within the community? We are seeking a dedicated and passionate Advance Practitioner to join our Locality Team in Leeds. This role is vital in providing high-quality care and support to individuals and families, ensuring their needs are met with compassion and professionalism.

As an Advance Practitioner, your key responsibilities will include:

Leading and supervising a multidisciplinary team to deliver effective care solutions. Conducting comprehensive assessments and developing tailored care plans for clients. Providing expert guidance and support to team members, fostering a culture of continuous improvement. Engaging with community resources and stakeholders to enhance service delivery. Ensuring compliance with regulatory standards and best practices in care provision.

To be successful in this role, you must possess the following essential qualifications and skills:

A relevant degree in Social Work, Nursing, or another health-related field. Registered with a professional body (e.g., Social Work England, NMC). Significant experience in a social care or healthcare setting, with a strong understanding of local services. Exceptional communication and interpersonal skills, with the ability to build rapport with clients and colleagues. Strong leadership skills, with a proven ability to mentor and support team members.
