Your key responsibilities will include:
Conducting comprehensive assessments of clients’ needs and circumstances. Developing and implementing individualized care plans in collaboration with clients and other professionals. Providing ongoing support and monitoring to ensure effective service delivery. Working closely with multidisciplinary teams to facilitate access to resources and services. Maintaining accurate and up-to-date case records and documentation. Participating in regular supervision and professional development opportunities.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW). Current registration with Social Work England. Strong knowledge of relevant legislation and frameworks, including safeguarding policies. Excellent communication and interpersonal skills, with the ability to build rapport with clients.
