Join our dedicated team as a Senior Practitioner in the Transitions Service in East Sussex, where you will play a crucial role in supporting young adults as they navigate the transition from childhood to adulthood. As a Senior Practitioner, you will be responsible for:
Leading assessments and developing care plans tailored to individual needs. Collaborating with multi-disciplinary teams, families, and external agencies to ensure a holistic approach to care. Providing guidance and supervision to junior staff, promoting best practices in social work. Monitoring and reviewing care packages, making adjustments as necessary to improve outcomes. Ensuring compliance with relevant legislation and safeguarding policies.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current Social Work England registration. Significant experience working within transitions or similar settings. Strong assessment and analytical skills, with a person-centered approach.
