As an Advanced Practitioner, you will lead on peer development and practice, supports and supervise staff to develop their skills and knowledge in line with related legislation including The Care Act and Hertfordshire's Connected Lives principles to contribute to the ongoing development of the service.

Advanced Practitioners are responsible for delivering high quality statutory social care including recognising safeguarding, delivering assessment, supervision and care management functions as well as developing innovative, creative community solutions.

Advanced Practitioners are pivotal in developing links with the local community through undertaking multi-agency work with partner organisations, providers, professionals, individuals, families, and carers.

There is responsibility for ensuring that all statutory health and social care responsibilities are met and delivered to a high standard including safeguarding, assessment, supervision, and care management functions.

Job Summary
Main Duties & Responsibilities
As the Advanced Practitioner for the Early Intervention Service you will be responsible for a team of Community Care Officers who complete the following role;

EIV is an Integrated Service operated by Health and Social care practitioners who can provide urgent assessments and interventions to patients at risk of hospital admission in their usual place of residenceThe role will involve making urgent social care assessment and interventions to support individuals to remain safe in their normal place of residence in the community or in supported living. This is completed through specialist assessment, equipment and review by health professionals. Predominantly of adults who have fallen at home and need support to be made safe at home rather than be taken to hospital by ambulanceThe Early Intervention Vehicle operates from Bricket Wood, Watford.A EIV CCO will complete specialist assessment in the community with health partnersEIV works closely with our health providers to ensure people get the right service to meet their needsEIV practitioners will require experience or undertake training as functional assessorsEIV work closely with private and voluntary sector .The successful candidate will support individuals to access community and voluntary supportEIV works closely to support unpaid and informal cares and will provide advice and support to cares who may need services to support them in their caring roleEIV is an inclusive service that works with individual from all cultures and backgrounds and provide cultural services and support to meet individual needs

Part 3 - Person Specification
Essential Desirable
Person Specification You'll be a qualified social worker or occupational therapist with a minimum of 2 years post qualifying experience. In addition, you will also need the following: Actively undertakes a specialist role i.e. Moving and Handling Practitioner Practice Educator Approved Mental Health Practitioner Best Interest Assessor Or agreement to undertake a specialist role within the first year of the role
Personal Attributes
Special Conditions Candidates must have full driving licence and access to own vehicle
