Your key responsibilities will include:
Conducting comprehensive assessments of adult service users to identify their needs and strengths. Developing and implementing person-centred care plans in collaboration with service users and their families. Providing ongoing support and guidance to individuals, helping them to navigate challenges and access resources. Working closely with multidisciplinary teams, including health professionals, to ensure holistic care delivery. Maintaining accurate and up-to-date records in accordance with social care regulations and policies.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly in community settings. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
